Here at Arohanui Hospice we strongly believe in providing great education and resources to benefit those who are tending to Palliative Care patients.
We want to encourage any Doctors and Nurses who have not attended this study day to consider doing so. It will enhance clinical practice and palliative care patient outcomes within your practice.
It is free to attend and when you become an accredited provider on the partnership there is funding available to meet the palliative care needs of your patients.
The Palliative Care Partnership (PCP) was established in Mid Central in 2004 to ‘provide enhanced palliative care services through quality coordinated health care using both specialist and primary health care services’. This is a formal partnership between Arohanui Hospice and General Practice Teams with the support of THINK Hauora to provide an integrated model of care which aims to ensure equity for the people of the Mid Central District to access a high-quality palliative care service wherever they may choose to be.
Our vision is ‘To maximise the skills and expertise of both primary and specialist services to provide the best possible palliative care in the community’
To care for those living with terminal illness can be challenging but at the same time very rewarding. Many general practitioners, nurse practitioners and practice nurses have said this because they find that it brings together the best in the clinical and holistic dimensions of primary care. The special bond that is often developed with patient and family at that time enables you to really feel that you are able to make a difference in their lives.
When working with those living with terminal illness the challenge is for all providers to deliver the best possible care, and the Palliative Care Partnership (PCP) is an attempt to maximise the skills and expertise of both primary and specialist services. The education programme is designed to improve General Practice Teams (GPT) knowledge and confidence in providing palliative care.
